一、本案為修正上開要點申請附件格式，其修正內容說明如下：
(一) 附件一：
１、領據暨切結書申請人資料，新增「申請人職稱」一列。
(二) 附件二：
１、獎勵經費請領清冊欄位，新增「職稱」一欄。
２、檢附資料檢核表，刪除「機關學校、事業機構之自行收納款項收據」及「機關學校、事業機構之帳戶資料」項目。
二、修正後「桃園市政府鼓勵所屬機關學校及事業機構員工提升客語能力作業要點」申請附件詳如附件。
