(一)學期成績（上學期）：國中60分以上；操行成績：80分以上（無操行分數者以老師評語為主）。國中生須附向學校申請有分數之成績單，否則不予受理。
（註：申請人數超過錄取名額時，以成績高低排序錄取。）
(二)領有縣市政府或鄉鎮市區公所低收入戶、中低收入戶證明。
申請時間:104年2月20日起至3月8日止，可親送 (收件時間：09:00～17:00)，或掛號郵寄
(本獎助學金須自行申請不受理團體申請)
