| 一、 | 依據財團法人嘉新兆福文化基金會115年4月28日嘉基綸字第1150000008號函辦理。 |
| 二、 | 體育獎學金分為學生獎學金、教練獎、及特別獎三類，申請期間自5月11日至6月11日止，申請資訊摘要如下： |
| (一) | 學生獎學金: 採線上申請，申請人請於相關網站辦理: https://application.chf.ngo/award/sports，系統開放時間為5月11日上午9時至6月11日下午17時止。 |
| (二) | 教練獎及特別獎：採紙本申請，截止日為6月11日(以郵戳為憑)。 |
| (三) | 申請表單可至基金會官網(網址：https://www.chf.ngo)『中華嘉新體育獎學金』自行下載。 |
| 三、 | 其餘相關申請資訊請參閱中華嘉新體育獎學金申請辦法（詳附件），個人資料依個人資料保護法及其施行細則等相關規定辦理。 |
| 四、 | 檢附申請辦法及相關表單共6份。 |
