| 一、 | 本簡章由「111學年度中等以上學校運動成績優良學生升學輔導委員會」第2次委員會議通過，並經教育部111年2月25日臺教授體字第1110007021號函核定，紙本簡章將另行郵寄。 |
| 二、 | 請各校承辦人員至本項試務網站查詢學校代碼(即帳號)，以帳號、密碼登入後始得進行網路報名，預設密碼為「S+學校代碼」，登入後請務必更新密碼，以維資訊安全，報名系統開放時間為111年4月13日上午9時至111年5月3日下午5時。 |
| 三、 | 報名程序經各校初審後，並完成核章，考生應繳之各項表件個別裝袋，連同審查費用匯票(各校可依總額購買一張，收據請留存)，另附貼足限時掛號回郵大型信封一個，詳填收件單位及通訊處，裝入大型信封，於報名申請截止日前(中華郵政郵戳為憑)，以限時掛號郵寄至404臺中市北區雙十路一段16號，「111學年度中等以上學校運動成績優良學生升學輔導委員會」收；逾期未送者，不得申請補辦。 |
