| 一、 | 依據適性入學委員會112年5月12日草商適性字第1120200138號函辦理。 |
| 二、 | 適性入學委員會考量部分國中畢業典禮舉行時間晚於各入學管道之錄取報到日，學生報到尚無學歷證件繳交，爰提供錄取報到切結書格式(如附件)。貴校如符合前揭情形，請協助轉知學生相關資訊，以利學生檢具文件至錄取學校完成報到程序。 |
| 三、 | 另因多項入學管道報到日期集中於112年6月15日(星期四)至6月16日(星期五)，請貴校確實告知學生若重複錄取不同管道，僅可擇一報到，以避免招生學校上傳至國立臺灣師範大學心理與教育測驗研究發展中心之報到名單無法勾稽，致影響後續免試入學招生名額控管作業。 |
