| 一、 | 依據國教署111年3月14日臺教國署高字第1110031353號函辦理。 |
| 二、 | 有關變更就學區注意事項如下： |
| (一) | 若學生有變更就學區需求者，貴校須至旨揭系統平臺協助學生完成送件。 |
| (二) | 為使各校作業方式統一，請貴校以電子公文函送「申請名冊」、「申請表及相關證明文件」等資料掃描檔至欲變更之就學區免試入學委員會審核，並副知原就學區免試入學委員會；相關文件之正（紙）本另行寄送。 |
| (三) | 如欲變更就學區之學生，未經貴校集體報名111年國中教育會考，則請學生逕行將申請表件送至欲變更之就學區免試入學委員會，貴校無須發函。 |
| 三、 | 若有相關疑義，請逕洽桃連區高級中等學校免試入學委員會（南崁高中），電話03-3525580分機212或217。 |
