| 一、 | 旨揭相關注意事項已公告於本校首頁及科學班首頁，請各國中學校協助轉知學生及家長。 |
| 二、 | 請詳閱「國立嘉義高級中學110學年度科學班甄選入學因應嚴重特殊傳染性肺炎(COVID-19，武漢肺炎)疫情甄選生及家長注意事項」(如附件1)。 |
| 三、 | 110年3月4~5日(星期四~五)至本校現場報名者，須配戴口罩、測量體溫(若有發燒，禁入校園報名)，並繳交「因應嚴重特殊傳染性肺炎(COVID-19，武漢肺炎)健康關懷表」(自行下載附件1-1或1-2)，才能入校現場報名。 |
| 四、 | 報名參加科學能力檢定甄選學生，請下載附件1-3「國立嘉義高級中學110學年度科學班甄選入學因應嚴重特殊傳染性肺炎(COVID-19，武漢肺炎)疫情切結書」及附件1-4「因應嚴重特殊傳染性肺炎(COVID-19，武漢肺炎)健康關懷表(甄選生)」務必隨報名文件一併繳交至本校。 |
| 五、 | 應試當日請應試學生務必全程配戴口罩並配合量測體溫，為落實防疫工作及減少人潮聚集，除試務人員及甄選生外，不開放甄選生親友進入校園內陪試。 |
| 六、 | 身心障礙、突發傷病甄選生可申請1位親友陪試，請填陪試申請書暨同意陪試通知書(如附件1-5)，於報名期間110年2月22日至3月5日掛號郵寄本校申請，經審核同意者由本校以電子信件寄送，陪試人員於甄試當日攜帶「陪試申請書暨同意陪試通知書」、本人身分證件並填寫健康關懷表(如附件1-6)，經工作人員查驗後，始進入校園內陪試，陪試人員請配戴口罩、量測體溫(若有發燒，禁入校園陪試)，儘可能待在開放空間。如屬居家隔離、居家檢疫或自主健康管理(被限制不可外出)者，不得陪試。 |
| 七、 | 提醒甄選生，應試前，若被衛生主管機關列管為「自主健康管理」(無症狀)對象，請於接獲通知後，立即與本校試務中心聯絡(連絡電話05-2762804轉204~205)，試務中心將協助安排甄選生至「備用試場」應試。 |
