| 一、 | 依據財團法人罕見疾病基金會附設新竹縣私立關西身心障礙福利服務中心113年8月28日罕病福字第1131150039號函辦理。 |
| 二、 | 旨揭中心除服務罕見疾病病友及其家屬外，亦透過基因教育中心推廣罕見疾病知識，增進國中小師生對罕病的認知，學習包容與同理生命教育，進而尊重與接納罕見疾病病友及家庭。 |
| 三、 | 旨揭參訪活動開放時間為每週一到週六上午9時至下午5時，採預約參訪方式(中心網址如下：https://www.tfrd2.org.tw/welfare/)，請本權責核予貴校出席人員公(差)假登記，有相關疑問可洽聯絡人陳威廷先生，電話：03-5475528轉1007。 |
